Umbrella Mental Health is a trade name (DBA) used by Jonathan Kim, PMHNP-BC, a sole proprietor licensed in the State of California. This practice is not a corporation, LLC, or partnership. All clinical services are provided solely by Jonathan Kim, PMHNP-BC.
Scope of services
Umbrella Mental Health provides outpatient psychiatric services via telehealth, including psychiatric evaluations, medication management, treatment planning, and supportive psychiatric care. All services are provided by Jonathan Kim, PMHNP-BC.
Psychotherapy, crisis intervention, inpatient care, and emergency services are outside the scope of this practice. If a level of care beyond outpatient psychiatry is needed, referral assistance will be provided.
Controlled substances
Controlled substances may be prescribed when clinically appropriate and when consistent with current standards of care. Prescribing decisions are made based on a complete clinical assessment — no medication is guaranteed in advance of evaluation.
Patients receiving controlled substances are expected to comply with prescription monitoring requirements, use a single pharmacy, and not seek the same controlled substance from other providers. Failure to comply may result in discontinuation of controlled substance prescribing or termination of care.
Controlled substances are not prescribed at a first appointment, or on the basis of self-reported history alone without corroborating documentation.
Financial policy
Insurance billing
Accepted plans include Aetna, Cigna, Quest Behavioral Health, Carelon Behavioral Health, and Anthem Blue Cross California. Copay, coinsurance, or deductible is due at the time of service. Any balance not covered by insurance remains the patient's responsibility.
It is the patient's responsibility to verify that their specific plan covers outpatient psychiatric services and that this practice is in-network. Coverage varies even within the same insurance carrier.
Self-pay
If you are not using insurance, self-pay rates are available. Payment is due at the time of service. Contact for current rates.
Outstanding balances
Unpaid balances may result in suspension of services until the balance is resolved. Payment arrangements may be available when needed.
Cancellation and no-show policy
Please provide at least 24 hours' notice to cancel or reschedule an appointment. This allows the time to be offered to another patient.
Appointments cancelled with less than 24 hours' notice, or missed entirely without contact, may be subject to a late cancellation or no-show fee. This fee is not covered by insurance and is the patient's responsibility.
Repeated no-shows or late cancellations may result in discharge from the practice.
Communication
Routine questions and non-urgent communication should be sent through the patient portal or by email. Every effort is made to respond to messages within two business days.
Clinical advice, diagnoses, or medication adjustments are not provided by email or phone between appointments. If your needs are urgent, call. If you are in crisis, call 988 or go to the nearest emergency room.
Friend requests or clinical communication through social media are not accepted.
California location requirement
Jonathan Kim, PMHNP-BC, is licensed in the State of California. California law requires that patients be physically located within California at the time of each telehealth appointment. Appointments cannot be conducted with patients who are outside of California during a scheduled visit, regardless of where they reside.

Good Faith Estimate — No Surprises Act
Under the No Surprises Act (effective January 1, 2022), you have the right to receive a Good Faith Estimate of expected charges for services before your appointment — if you are uninsured or if you choose not to use your insurance.
The estimate will include anticipated costs for services expected to be provided. It is not a contract, and actual costs may vary based on what occurs during treatment. You may dispute a bill that exceeds your Good Faith Estimate by $400 or more.

Termination of care
Either party may end the clinical relationship at any time. If care needs to be ended (for example, due to non-compliance, safety concerns, or inability to meet your clinical needs), reasonable notice will be provided along with referral assistance to ensure continuity of care.
