Your Privacy Choices

Umbrella Mental Health is a trade name (DBA) used by Jonathan Kim, PMHNP-BC, a sole proprietor. This practice is not a corporation, LLC, or partnership.

Request access to your information

If you have submitted information through this website — such as a contact form or appointment request — you may request access to, correction of, or deletion of your information, where applicable.

Verified requests will be responded to within a reasonable timeframe, typically within 30 days.

To submit a request, please email info@umbrellamentalhealth.com with the subject line "Privacy Request." Include the name and email address used so the information can be located.

Request a correction

If you believe information you've submitted is inaccurate, you may request a correction. Include the details of what needs to be corrected and what the correct information is.

Send correction requests to info@umbrellamentalhealth.com with the subject line "Privacy Correction Request."

Request deletion

You may request deletion of contact information you've submitted, where retention is not required by law or legitimate business interest. You will be notified of any portions that cannot be deleted and why.

Note: deletion requests apply to website-submitted contact information only. Medical records are subject to different retention requirements under California law and HIPAA. For medical records inquiries, contact directly.

Opt out of analytics

This site may use web analytics tools (such as Google Analytics) to collect anonymized usage data. This data does not identify you personally.

Response timeline

Privacy requests will be acknowledged within 5 business days and completed within 30 days. If more time is needed, you will be notified of the extension and the reason.
